Comprehensive Primary Care Medical Services in Los Angeles and San Bernardino

At St. John’s Community Health, services are provided in a medical home setting. Each patient is assigned a primary care physician and care team who work with the patient to address their health care needs and priorities.

We provide a wide range of medical services in Los Angeles and San Bernardino from routine preventive care to management of chronic diseases such as diabetes and high blood pressure.

Our Medical Services Include:

  • Wellness visits (physical exams) for all ages

  • COVID-19 testing, vaccination, triage and follow-up care

  • Immunizations

  • Screening and treatment for chronic medical conditions (such as asthma, diabetes, high blood pressure)

  • STD and HIV Testing and treatment

  • Vision and hearing services

  • Cardiology

  • Laboratory and Pharmacy

  • Nutrition counseling

  • Transgender health care

  • Podiatry

  • Optometry Services including eye exams, retinal screenings, eye dilation, visual field testing, OCT scanning that screens for glaucoma and eye macula, dispensing of frames/glasses. Full optometry services are offered at our clinic at 808 W. 58th Street, Los Angeles, CA 90037

Medical Home Setting Services

Our medical services in Los Angeles and San Bernardinoare provided to clients in a medical home setting using evidence-based care. St. John’s Community Health has been recognized by the National Center for Quality Assurance as a Patient-Centered Medical Home (PCMH), which is a model that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.

The PCMH model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. The PCMH model has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.

Patient Resources

At St. John’s, anyone is eligible to receive services, no matter their ability to pay. We accept Medi-Cal, Medicare and many other insurance plans. We have benefits counselors at all of our health centers to help patients determine their eligibility for insurance programs. For uninsured patients we offer services on a sliding scale cash basis. We offer Pharmacy, prescription and Laboratory services including: Senior programs, Public-Private Partnership, Sliding Fee Scale Pharmacy Programs, diagnostic and laboratory procedures, HIV and STD testing and more. Our Dispensaries offer prescription services to enrolled clinic patients.

Tranquada, MD Safety Net Initiative, which provides funding opportunities to safety net providers throughout Los Angeles County to improve their core infrastructure and capabilities and to support projects that have a long-term and systemic impact on health equity.
