At St. John’s Community Health, services are provided in a medical home setting. Each patient is assigned a primary care physician and care team who work with the patient to address their health care needs and priorities.

Medical Home Setting Services
Our medical services in Los Angeles and San Bernardinoare provided to clients in a medical home setting using evidence-based care. St. John’s Community Health has been recognized by the National Center for Quality Assurance as a Patient-Centered Medical Home (PCMH), which is a model that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.
The PCMH model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. The PCMH model has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.

Care’s Elevating the Safety Net Initiative focused on strengthening L.A.
